Nearly a year after the high-profile acquisition of the former Pier 1 Imports building, NYU Langone Health is moving forward with the transformation of 54 Montauk Highway into a state-of-the-art medical facility. The nearly 20,000-square-foot site, one of the largest commercial properties on the East End, is currently undergoing the comprehensive renovation required to transition the space from a retail warehouse to a multidisciplinary healthcare hub.
Once completed, the Water Mill facility will serve as a centralized destination for imaging services and physician practices. This model is designed to streamline the patient experience by allowing East End residents to consult with specialists and receive diagnostic imaging in a single visit. The project utilizes the building’s significant footprint to provide over 100 parking spaces and improved access via an easement from Flying Point Road, addressing the high-traffic demands of the Montauk Highway corridor.
This development is a key pillar in NYU Langone’s aggressive East End expansion. Over the past year, the health system has significantly fortified its regional presence with several major milestones. The 6,000-square-foot Medical Associates office in Westhampton Beach is now fully operational, offering primary care, cardiology, and endocrinology. Further east, a new 25,000-square-foot world-class imaging and specialty center opened in Riverhead in March 2026, offering advanced diagnostics alongside orthopedic and vascular care.
In addition to these ambulatory sites, the health system has fully integrated NYU Langone Hospital—Suffolk in Patchogue as its seventh inpatient facility. A modern ambulatory surgery center on Patchogue’s Main Street is also set to open this summer, further expanding outpatient surgical options for the community. The Water Mill project represents a $10.85 million investment in local infrastructure, repurposing a prominent site that had sat vacant since 2019 to provide a permanent solution for the growing healthcare needs of the South Fork.
As the renovation and interior build-outs continue through 2026, the facility is poised to become a critical link between NYU Langone’s existing practices. By converting a former nightclub and retail space into a long-term clinical asset, the health system is ensuring that world-class care remains highly accessible to East End residents.
